The medium size is the standard size for most users. This size fits the average size nose with average nostrils. This size will fit larger noses with elongated nostrils. You can compare it to the end of a sharpie marker if desired. If you would like to measure your nostril opening across (not front to back). The measurement if desired is 15mm or aobut 5/8 of an inch.
